My fee is $100 per 50 minute session.
Insurance and Managed Care Panels
If you plan to use your mental health coverage, before the first appointment call the "Mental Health and Substance Abuse" number on the back of your card and ask for pre-certification. Bring the authorization number to your appointment. Also determine your co-pay and if there is an unmet deductible.
My psychotherapy services are covered as in-network by the following managed care plans:

I am covered by PPO plans as an out-of-network provider.

To save time, print and fill out the intake form and bring to your first appointment. If a couple, each partner fills out their own form. Each form is available as a PDF.

There are advantages to paying for psychotherapy without using insurance coverage.
Filing a health insurance claim requires me to submit a mental health diagnosis for the client. A mental health diagnosis may have future effects on insurability for life insurance or qualification for sensitive positions.
In addition, managed care requires that I submit treatment reports with highly personal information about your private matters the confidentiality of which I cannot guarantee.
